Entry clearance is a standing issued by a British Mission, akin to an Embassy, High Fee or Consulate, outside of the UK. It cannot be issued inside the UK. Entry clearance grants the applicant (on this case the surrogate child) to journey to the UK in a specific capability. The visa takes the type of both a stamp in a overseas passport or a freestanding journey doc if the kid is stateless.
